Caregiver Name Tag and Dress Code Policy

Our personal appearance is a direct reflection of the Company. We expect everyone to be neat and well groomed at all times and to maintain an appearance appropriate to the position held and consistent with presenting the best image of the Company to customers, government officials, and visitors. Please see the Handbook for more information.

Pay Schedule Overview

For payroll-related questions, please contact the Payroll Manager, Madison Fariss.

  • Employees are paid twice per month, on the 10th and the 25th.

  • Each month is divided into two pay periods: 1st–15th and 16th–end of month.

  • Time worked from the 1st through the 15th is paid on the 25th of the same month.

  • Time worked from the 16th through the end of the month is paid on the 10th of the following month.

Well Sky Mobile App User Guide

All caregivers must use the Well Sky App to clock in and out and obtain client signatures after each work shift. Failure to properly clock in and out and obtain client signatures after each work shift may result in discipline, up to and including termination of employment.

Charting must be accurate and up to date. Caregivers can request changes to task list and update care plans.

Telephony Log in is optional when having problems with app.

Time Off Requests

Due to the nature of our business, unplanned or unexpected absences that leave shifts uncovered may result in undue hardship for our clients and their families. The Company will apply the following progressive discipline for call-in absences per quarter (every three months):

  • After 3 call-ins: Verbal warning issued

  • After 4 call-ins: Written warning issued

  • After 5 call-ins: Corrective action, up to and including termination

  • No call, no show: Termination

The Company reserves the right to advance the level of corrective action depending on the nature of the violation and the occurrence of previous corrective actions. Any incomplete shift, early departure, or late arrival may be considered a call-in.

Call-In Procedures

If you wish to request specific day(s) off, a Time Off Request Form must be submitted to the office at least two (2) business days prior to the requested date(s) and must be approved by the office.

  • Requests for 3–6 consecutive days off must be submitted at least 15 business days in advance.

  • Requests for 7 or more consecutive days off must be submitted at least 30 days in advance.

All time off requests require office approval.

Time off requests submitted via voicemail, social media, or any other non-approved method will not be honored.

Break Time

  • If you are required to take your meal period while on assignment with a client, you must do so at a time that does not interfere with your client care responsibilities.

  • Because meal periods are taken as client care obligations permit, you will be paid for your meal period while on assignment with a client. No meal deductions will be made for meals taken during client assignments.

  • Running personal errands while on patient time constitutes Medicaid fraud and may result in severe penalties, including fines and imprisonment.

  • If you need to take a break for personal reasons, please clock out and notify a scheduler.

Weapons

Pursuant to and in compliance with Kentucky law, the company does not prohibit employees from possessing legally owned weapons locked inside of the employee’s privately owned vehicle.

The company does, however, strictly prohibit the possession of any and all weapons on company premises, including in company owned vehicles, client homes other than as specified above, in accordance with applicable law and the company’s weapon policy.

Caregiver Certification Requirements

To remain active and eligible to work, all caregivers must keep the following certifications and documents current. Failure to maintain active requirements may result in removal from the schedule.

TB Skin Assessment & Daily Test

  • Expires annually (every 1 year).

  • TB Skin Assessment/Daily Test and your annual evaluation will be sent to you each year while employed.

  • A physical TB test is not required. You will complete the provided assessment and submit it as instructed.

CPR / First Aid

  • Expires every 2 years.

  • Allied will pay for your test if renewal is required.

  • If your certification is current, you may upload your active certificate.

  • You may not work with an expired CPR/First Aid certification.

  • Failure to complete renewal on time will result in removal from the schedule until you are active again.

Auto Insurance

  • Expires at your policy renewal (every 6 months to 1 year).

  • Required to claim mileage.

  • Caregivers listed as non-drivers are not required to carry auto insurance.

  • Transportation of clients is not permitted unless auto insurance is active.

Driver License / Photo ID / Social Security Card / Passport

  • Expires at your renewal date. Temporary cards are acceptable.

  • Required to claim mileage and to transport clients.

  • You may not work with expired identification.

  • If you do not have a driver license, you must provide a valid photo ID and keep it active while employed.

  • Transportation of clients is not permitted unless both auto insurance and a valid driver license are active.
