Allied Care provides patient-centered case management services for individuals who need guidance coordinating supports, developing care plans, and accessing resources. Our role is to assist you in understanding your goals, help you build a plan that fits your needs and preferences, educate and support you in making informed choices, and stay connected with you as those needs change.

What Case Management Includes

Personalized Plan of Care

Allied Care creates a care plan tailored to your needs, goals, and daily living requirements. We coordinate with families, providers, schools, and care teams to ensure the plan supports every part of your life.

Monthly Check-Ins

We stay in touch each month to review progress, update your plan, and confirm that services are working well. If something needs to change, we adjust your plan quickly.

Incontinence Supplies

For eligible clients, Allied Care helps obtain incontinence supplies through the Goods and Services program and ensures they are delivered consistently.

Ongoing Support and Coordination

We communicate with caregivers, providers, and other team members to keep services aligned, organized, and reliable.

Traditional Services and PDS

Allied Care provides case management for both Traditional Services and Patient Directed Services (PDS). We help individuals and families understand their options and choose the approach that best fits their needs and preferences.

Non-Medical Goods and Services

Allied Care helps eligible individuals access non-medical goods and services approved by their primary care provider, including home and environmental modifications that support safety and independence, such as wheelchair ramps, walk-in showers, and mobility equipment.

Who Case Management Supports

Case Management is designed for individuals who need help navigating services, coordinating care, or accessing resources such as medical appointments, community programs, or daily living supports. This includes children, adults, and seniors with a wide range of disabilities.

How to Qualify

Services can be funded through the Medicaid HCB Waiver, Private Pay, or VA programs where authorized.
