Waiver Applications

We guide your waiver application from start to finish, handle all submissions, and keep you updated every step of the way.

Home Care Services

We provide personal care, homemaking, meal prep, laundry, medication reminders, and errands to keep our clients safe at home.

Case Management

A dedicated specialist creates your care plan, completes monthly check-ins, and helps you secure incontinence supplies if you qualify.

Adult Day

We offer supervised daytime programs for adults 21+ with peer interaction, social activities, and medical oversight.

How Services Are Funded

We support several pathways to begin services.

    We coordinate with case managers and begin services once approvals are in place.

    Flexible scheduling and customized support with transparent pricing.

    For eligible Veterans and beneficiaries, we work with the VA to obtain authorizations where available.

    Kentucky Transitions
    A Medicaid program that allows nursing home residents to transition back into the home and have funding for our services.
